[PATCH 3.4 00/45] 3.4.104-rc1 review

This is the start of the stable review cycle for the 3.4.104 release.
There are 45 patches in this series, all will be posted as a response
to this one. If anyone has any issues with these being applied, please
let me know.
Responses should be made by Thu Sep 25 02:03:31 UTC 2014.
Anything received after that time might be too late.
A combined patch relative to 3.4.103 will be posted as an additional
response to this. A shortlog and diffstat can be found below.
